چکیده

3. itral, 33%) and cases of tricuspid endocarditis are rare (2%). This because of the anatomical relationship with the atrioventricular de, which is situated at the centre of Koch’s triangle, close to the ncoronary aortic leaflet and the anterior mitral leaflet; the icuspid septal leaflet, on the other hand, is located at the base of ch’s triangle. In a study of patients with endocarditis and mplete AVB, pathology workup revealed the presence of an fection, usually accompanied by abscesses and fistulas, that fected the conduction pathways; in cases involving paroxysmal B, inflammation was observed at this level, which would plain the reversibility of the event. Electrocardiographic onitoring is important in patients with endocarditis, as the pearance of conduction abnormalities can alert us to the pearance of perivalvular complications; similarly, once a nduction block has been detected, its disappearance in less an a week indicates a good response to antibiotic treatment. Right-sided endocarditis is an underdiagnosed condition, obably due to its atypical clinical presentation, although getations can be readily observed even by transthoracic hocardiography. The case described here is exceptional in at it involved isolated tricuspid endocarditis appearing with mplete AVB and left bundle-branch block that was reversed on treatment with antibiotics. Cases of conduction abnormales in association with tricuspid valve endocarditis have rarely en described in the literature, although there appears to be a ndency for the block to be reversed with medical treatment, 4.

Percutaneous retrograde transfemoral closure of mitral paravalvular leak in 3 patients without construction of an arteriovenous wire loop.

Percutaneous closure of paravalvular leaks has emerged as an alternative to repeated surgeries. Different percutaneous techniques and various devices have been used, off-label, for paravalvular leak closure. For mitral leaks, antegrade transseptal, retrograde transfemoral, and retrograde transapical techniques have been developed. In the antegrade transseptal approach, an arteriovenous guidewir...

Failed percutaneous closure of a mitral prosthesis paravalvular leak.

A 77-year-old man who had a mechanical prosthetic mitral valve and aortic valve implanted in 1993 presented dyspnea and anemia, and was diagnosed of mitral prosthesis paravalvular leak. The patient was considered at high operative risk owing to his age, severe pulmonary hypertension, chronic renal failure, and previous cardiac surgery. Surgical treatment in his reference hospital was therefore ...

Percutaneous Closure of Mitral Paravalvular Leak Guided by Three-Dimensional Transesophageal Echocardiography

Reoperation is the method of choice for correcting prosthetic paravalvular leaks. The percutaneous repair of mitral paravalvular leaks might represent an interesting alternative in select cases. The present study reports two cases of mitral paravalvular leaks that were treated with AmplatzerTM Vascular Plug III guided by three-dimensional (3D) transesophageal echocardiography, using the percuta...

Paravalvular leak closure with the Amplatzer Vascular Plug III device: immediate and short-term results.

INTRODUCTION AND OBJECTIVES Percutaneous paravalvular leak closure is a complex procedure with varying success rates; the lack of closure devices specifically designed for this purpose has hampered this technique. The characteristics of the Amplatzer Vascular Plug III appear to be well suited for paravalvular leak closures; however, the available data are limited to case reports or small series...

Percutaneous closure of prosthetic paravalvular leaks - should it be considered the first therapeutic option?

Paravalvular leak (PVL) is one of the most common complications of surgical implantation of mechanical or biological valve prostheses and, more recently, with percutaneous valve replacement. Its incidence ranges, according to the series, between 2 and 10% for aortic valves and 7 and 17% for mitral valves. Although most patients with PVL have few or no symptoms and are accordingly treated conser...

Percutaneous closure of an aorta to left atrium fistula with an Amplatzer duct occluder.

We describe percutaneous closure of an unusual paravalvular leak, with a communication from the noncoronary cusp of the aortic valve to the left atrium adjacent to a St. Jude mitral valve prosthesis, in the absence of an infection. The patient presented with worsening dyspnea and edema. The anatomic location of the valve annulus adjacent to the noncoronary cusp underlies the etiology of this co...
